The program, called CHOICES, is run by Christ Community Health Services in Memphis, TN. CHOICES is the second of two innovative programs Christ Community started that’s garnering national recognition. The Agency for Healthcare Research and Quality also recognized Christ Community’s Memphis Healthy Churches programs on its Innovations Exchange Web site.
